2) Not Just Going with Your Gut: IBD Transition

Poll the audience to identify common barriers to ensure seamless transition to adult care
Identify a curriculum of important topics to address when transitioning IBD care
Share data regarding success of transition program by reviewing the results of our questionnaire
Provide a framework of how to establish a similar program at other centers
Establish stakeholders in ongoing growth of the project
Collaboratively refine and brainstorm ideas of resources/tools valuable in the transition process
